Checks Footnote in CALS table title

See Issue 70. The title of the following CALS table contains a footnote. We expect only one footnote in the generated HTML file.

I admit that this is a very strange use of footnotes. I think a table/caption should be used. But I actually encountered this use of footnotes when converting a Markdown document to DocBook.

We are not interested in the table itself, but in the placement of this footnote
